Role: Licensed Practical Nurse (LPN) delivering high-quality, tech-enabled patient care in a dynamic clinical environment.
Key Duties:
- Partner with RNs and physicians to provide hands-on clinical care.
- Administer medications and perform routine clinical tasks within scope of practice.
- Document assessments and care activities in advanced electronic health record systems.
- Support both in-person and virtual visits.
- Assist patients in using digital health tools to improve engagement and care experiences.
- Contribute to workflow and process improvement efforts.
Qualifications & Skills:
- Current LPN license and ability to practice within regulatory and clinical standards.
- Strong clinical skills, attention to detail, and safe medication administration.
- Proficiency with electronic health records and comfort with technology-enabled care.
- Excellent communication and collaboration skills with interdisciplinary teams.
