Work involves community outreach, coordinating needs during and after housing placement, supporting daily living skills, offering counseling, and advocating against discrimination and healthcare inequities. The team emphasizes member choice, harm reduction, non-coercion, and person-centered care. Follow-up support continues for months post-housing to ensure stability and independence. This field-based role requires flexible hours, on-call availability, and travel within three counties. Minimum requirements include a master’s degree in social work, mental health counseling, nursing, or psychology, with licensure or plans to obtain it. Full-time position with benefits, offering $32-$34/hour, and relocation to Norwich, NY is required before starting.
