The multi-disciplinary team includes a leader, clinicians, Care Managers, and Peer Specialists, utilizing the evidence-based Critical Time Intervention model.
Responsibilities include community outreach, coordinating needs before and after housing placement, enhancing daily living skills, offering supportive counseling, and advocating against discrimination and healthcare inequities.
The role emphasizes member choice, harm reduction, non-coercion, flexibility, and person-centered care. Post-housing, the team supports participants for months to ensure stability and independence.
This field-based position requires periodic on-call hours, flexible schedules, and travel across three counties. Candidates must hold a relevant master’s license or plan to obtain licensure, with experience working with homeless populations.
