Key responsibilities include providing
behavioral health assessments, counseling, and care coordination, collaborating with primary care providers, participating in
school and community outreach programs, and maintaining accurate documentation in the EHR.
Qualifications required are a
Master’s in Social Work (MSW) and an
LCSW license or eligibility. Prior experience in clinical or medical settings is preferred, along with strong communication and teamwork skills.
Schedule is
Monday–Friday in an outpatient clinic, offering a
