We are seeking a Part-Time LCSW specializing in Geriatric Psych for outpatient mental health services in long-term care facilities, currently in Manitowoc, WI.
Provide psycho-social assessments, diagnosis, and treatment, including individual, group, and family therapy for nursing home residents.
Collaborate with an interdisciplinary team to ensure high-quality, continuous care and adherence to Medicare guidelines.
Manage a flexible caseload of 10-15 patients daily, with support from a dedicated practice manager.
Utilize electronic medical records and maintain professional documentation.
Qualifications include an active LCSW license, 1-2 years of experience, excellent communication skills, and proficiency with technology.
Preferred experience in healthcare settings and Medicare standards is a plus.
