Licensed Clinical Social Worker (LCSW)

We are hiring a part-time LCSW Geriatric Psych Social Worker to provide psycho-social services in long-term care facilities in Manitowoc, WI.

Responsibilities include conducting evaluations, diagnoses, and developing treatment plans with individual, group, and family therapies for nursing home residents.

Collaborate with interdisciplinary teams to ensure quality care and documentation compliant with Medicare guidelines.

Manage a flexible caseload of 10-15 patients daily, with autonomy and clinical support.

Utilize electronic medical records and coordinate with facility staff.

Qualifications include an active state LCSW license, 1-2 years’ experience, strong communication skills, and proficiency with technology. Experience in healthcare or nursing homes is a plus.
