We are seeking a part-time LCSW with geriatric behavioral health experience to provide psycho-social services in long-term care facilities in Manitowoc, WI.
Provide evaluations, diagnoses, and therapy (individual, group, family) for nursing home residents.
Collaborate with interdisciplinary teams to ensure quality and continuity of care.
Manage 10-15 patients daily with flexible scheduling for work-life balance.
Maintain knowledge of Medicare guidelines and documentation standards.
Qualifications include a Master’s degree, full state LCSW license, 1-2 years’ experience, and strong communication skills.
Proficiency with medical records and technology required.
