We are a growing provider of psychiatric and psycho-social services in long-term care, supporting over 600 facilities across multiple states. Currently, we have a part-time opening in Manitowoc, WI for a Geriatric Psych Social Worker (LCSW).
Qualifications include an LCSW license, 1-2 years’ experience, strong communication skills, and proficiency with technology.
Responsibilities involve providing direct psycho-social services, including evaluations, diagnoses, individual, group, and family therapies to nursing home residents, typically seeing 10-15 patients daily.
Work collaboratively with an interdisciplinary team, ensure documentation meets Medicare guidelines, and maintain a flexible schedule (M-F, daytime).
