Seeking a full-time Licensed Clinical Social Worker to provide high-quality mental health services within an integrated care setting.
**Responsibilities:**
Collaborate with a multidisciplinary team to assess, diagnose, and treat mental health and chronic conditions.Design and deliver evidence-based interventions, including brief therapy and crisis support.Coordinate care with community resources and ensure seamless patient access.Participate in quality improvement initiatives and maintain professional credentials.Contribute to clinic goals, perform duties across various locations, and support community needs.**Qualifications:**
