Seeking a Full-Time Licensed Clinical Social Worker (LCSW) to provide integrated mental health services in a community-focused health center.
Assess, diagnose, and treat mental health and chronic conditions using evidence-based interventions. Collaborate with a multidisciplinary team, including providing follow-up and facilitating referrals. Conduct screenings, develop treatment plans, and deliver brief individual and family therapy. Ensure high-quality, accessible care while meeting productivity and access goals. Participate in quality improvement, community outreach, and ongoing professional development. Maintain licensure and credentials, and work in person at Petaluma, CA, with bilingual Spanish skills required.
