Role: Registered Nurse (RN) in a hospice setting providing end-of-life care in patients’ homes.
Responsibilities:
- Deliver compassionate, patient-centered hospice care focusing on comfort, dignity, and quality of life.
- Conduct comprehensive patient assessments, manage symptoms and pain, and develop individualized care plans.
- Educate and support patients and families throughout the end-of-life journey.
- Ensure thorough, compliant documentation in electronic medical records (EMR).
- Travel to patient homes within designated territory to provide care.
Qualifications and Skills:
- Active RN license in the state of practice.
- At least two years of RN experience; hospice experience preferred.
- Current driver’s license with ability to drive approximately 20% of workday.
