Role: Provide compassionate, person-centered hospice care for individuals in home and community settings as a Licensed Practical Nurse.
Key Responsibilities:
- Deliver skilled nursing services under RN/physician direction.
- Perform nursing assessments and monitor patient status.
- Provide pain and symptom management and administer medications.
- Provide wound care and other direct patient care needs.
- Implement individualized care plans and support evidence-based, outcomes-focused practice.
- Document clinical findings and care delivered in the EMR.
- Educate patients and families on care, comfort measures, and support needs.
- Collaborate with an interdisciplinary hospice team to coordinate end-of-life care.
Qualifications & Skills:
- Current LPN licensure.
- Strong communication, reliability, and professionalism.
- Respect for diverse backgrounds and individual end-of-life preferences.
- Ability to provide empathetic care and work effectively in a team-based environment.
