Role: As a Hospice Field Case Manager, you will visit patients in community and facility settings to perform physical and psychosocial assessments, document visits electronically, develop nursing care plans, and make care recommendations based on patient needs.
Responsibilities: Conduct patient assessments, create and implement care plans, and provide care recommendations. You will also participate in a backup On Call rotation 2-3 times per month and work one weekend per month, with car and escort services available for On Call shifts.
Qualifications: Requires an Associates/Diploma in Nursing (BSN preferred), a NYS RN license, and a NYS Driver’s license. 2+ years of experience in med/surg, community health, or oncology nursing is preferred.
Skills: Strong assessment, care planning, and documentation skills, along with the ability to work independently in diverse settings.
