Hospice RN Case Manager provides compassionate, patient-centered end-of-life care in home and community settings.
Key responsibilities include:
- Manage a caseload of hospice patients
- Perform comprehensive patient assessments
- Develop, implement, and update individualized care plans
- Coordinate care and services with an interdisciplinary team
- Deliver expert pain and symptom management
- Educate and support patients and families
- Ensure seamless communication with physicians and caregivers
- Accurately document care in accordance with clinical standards
Qualifications/skills: Registered Nurse with strong clinical judgment, assessment and care-planning skills, proficiency coordinating interdisciplinary services, and the ability to communicate effectively with families and care teams while maintaining high standards of documentation and mission-driven, compassionate care.
