Role Overview:
Provides specialized nursing care to hospice patients and their families, focusing on symptom management, pain control, and holistic support. Responsible for assessing patient needs, developing and implementing individualized care plans, and collaborating with an interprofessional team to ensure quality and compassionate care. When acting as a case manager, coordinates all aspects of patient care across the continuum.
Key Responsibilities:
- Assess patient preferences, cultural, emotional, and spiritual needs.
- Develop, document, and communicate personalized care plans with clear goals.
- Promote respectful, equitable, and empathetic interactions with diverse populations.
- Evaluate patient condition changes, update care plans, and communicate with families and team members.
- Ensure evidence-based practices and quality outcomes in nursing care.
Qualifications & Skills:
- Nursing Diploma/Associate’s Degree in Nursing; Bachelor’s preferred.
- Valid RN license and driver’s license.
- Minimum 2 years of nursing experience; 2-5 years in home health or nursing preferred.
- Experience in case management is advantageous.
