Role: Hospice RN Admissions Nurse
Responsibilities:
- Conduct in-home hospice admission assessments and start-of-care visits.
- Evaluate patient eligibility and educate patients and families about hospice services.
- Develop initial care plans tailored to patient needs.
- Coordinate care with physicians and the interdisciplinary team.
- Support elderly and chronically ill patients in remaining safely at home.
- Utilize strong clinical judgment, compassionate communication, and accurate EMR documentation.
Qualifications & Skills:
- Registered Nurse (RN) license.
- Experience in hospice, palliative, or home health care preferred.
- Strong clinical assessment and communication skills.
- Ability to work independently in a field-based setting.
- Compassionate attitude focused on patient-centered care.
