Speech-Language Pathologist (Home Health)
Provide skilled home health speech-language pathology services by assessing patient needs and delivering evidence-based interventions to improve communication, swallowing, self-care, and overall quality of life.
Key duties
- Conduct patient evaluations and develop individualized treatment plans
- Implement therapeutic interventions to support communication and swallowing
- Provide education and training for patients and families to promote effective home strategies
- Coordinate care with patients, families, and interdisciplinary teams
- Document services and clinical findings in accordance with required standards
- Support quality improvement and patient safety initiatives
