Position Overview: A Physical Therapist (PT) is sought to assess, teach, and deliver therapy interventions in home health settings, collaborating with an interdisciplinary team to optimize patient outcomes.
Key Responsibilities:
- Perform initial and ongoing assessments of mobility, strength, and function.
- Develop and document plans of care, goals, and progress reports.
- Deliver physical therapy services per physician orders and modify treatment as needed.
- Manage discharge planning, including patient education on home exercises.
- Coordinate with healthcare team, document accurately, and supervise related personnel.
- Complete OASIS assessments where permitted by regulations.
Qualifications & Skills:
- Degree in Physical Therapy from an accredited program.
- Current, unrestricted state license.
- Proficiency in assessments, documentation, and compliance.
- Preferred: strong organization, communication, attention to detail, and experience with home health therapy and OASIS documentation.
