Position Overview:
We are seeking an Occupational Therapist (OT) to assess, teach, and deliver therapy interventions based on physician-directed home health plans. The role involves collaborating with interdisciplinary teams, performing assessments of mobility, strength, and function, and developing plans of care with measurable goals and outcomes. Responsibilities include documenting progress, modifying treatment plans, discharge planning, and educating patients and caregivers. The OT may supervise other personnel and complete assessments like OASIS where permitted by regulation.
Requirements:
- Degree in Occupational Therapy from an accredited program
- Current, unrestricted state license
- Valid CPR certification

Key Duties:
- Conduct assessments and interpret tests to establish goals
- Deliver therapy services per physician orders

- Coordinate with healthcare team and educate patients/caregivers
- Supervise other staff as applicable
