We are seeking a Home Health Registered Nurse to deliver patient-centered care in patients’ homes across Wisconsin.
Responsibilities include:
- Assessing patient needs and developing/updating care plans
- Administering treatments and monitoring patient progress
- Providing education to patients and caregivers
- Supporting recovery, chronic disease management, and hospice care
- Coordinating with families and interprofessional care teams
- Adapting to changing assignments as a float pool nurse
Qualifications and skills:
- Current Wisconsin RN license
- Reliable transportation
- Preferred relevant nursing experience
