Registered Nurse (Home & Community-Based Care)
Provide compassionate, client-centered skilled nursing in clients’ homes. Responsibilities include conducting nursing assessments, developing and updating individualized care plans, and administering medications and treatments. Coordinate care with physicians, therapists, and family caregivers to support safe, effective services.
Focus on promoting safety, independence, comfort, and dignity for seniors and individuals with disabilities. Serve as a reliable member of an interdisciplinary team, using clinical judgment to deliver high-quality care.
Key duties
- Perform assessments and monitor client health needs

Qualifications & skills
- Current RN licensure
- Strong clinical assessment and care-planning skills
- Medication administration and treatment delivery proficiency
