Position Overview
Seeking a Physical Therapist (PT) to join a home health care team, enabling patients to recover in their own homes. The role involves performing initial and ongoing assessments of mobility, strength, and function, and developing personalized treatment plans to achieve functional goals. Responsibilities include delivering physical therapy services per physician orders, maintaining documentation, modifying treatment as needed, and planning discharges with patient and caregiver education. The PT collaborates with healthcare team members, supervises relevant personnel, and ensures compliance with policies, regulations, and documentation standards, including OASIS assessments where permitted.
Qualifications and Skills:
- Degree in Physical Therapy from an accredited program
- Current, unrestricted state license
- Valid CPR, automobile insurance, and driver’s license
Preferred Experience: Home health therapy and OASIS documentation experience.
