Position Overview: A Physical Therapist (PT) provides home health care services, enabling patients to recover from illness or injury in their own homes.
Key Responsibilities: Conduct initial and ongoing assessments of mobility, strength, and function; develop and document personalized care plans with goals and outcomes; deliver therapy following physician orders; perform timely documentation to maintain plans of care; collaborate with healthcare teams, patients, and caregivers; supervise staff (PTA, OTA, HHA); complete OASIS assessments where permitted; ensure compliance with policies, standards, and regulations; and facilitate discharge planning with education on home exercises.
Qualifications & Skills: Degree in Physical Therapy from an accredited program; current, unrestricted state license; valid CPR, driver’s license, and auto insurance. Preferred: experience in home health therapy and OASIS documentation.
