Position Overview
Seeking a Full-Time Physical Therapist (PT) to join a home health care team, enabling patients to recover at home. The role involves performing assessments, developing and implementing treatment plans, and documenting patient progress.
Key responsibilities include:
- Evaluating mobility, strength, and function through assessments
- Administering and interpreting tests to set goals and outcomes
- Creating and updating plans of care, including discharge planning and home exercise education
- Delivering therapy services according to physician orders
- Coordinating with healthcare teams and documenting effectively to ensure compliance
- Supervising personnel such as PT Assistants and Home Health Aides where applicable
- Completing assessments like OASIS, adhering to policies, regulations, and standards
Requirements:
- Degree in Physical Therapy from an accredited program
- Unrestricted state license as a Physical Therapist
- Valid CPR certification
