Role Overview: A Home Health Physical Therapist Assistant provides personalized in-home physical therapy to help patients regain strength, mobility, and independence, focusing on functional improvement and quality of life.
Key Responsibilities:
Implement treatment plans developed by Physical Therapists
Perform therapeutic exercises, massage, traction, heat, cold, water, and electrical modalities
Document patient records and submit clinical documentation promptly
Participate in educational meetings and department in-services
Report equipment issues, supply shortages, or patient/staff changes
Apply infection control and universal precautions during patient care
Qualifications & Skills:
Current state license to practice Physical Therapy
Valid driver’s license, auto insurance, reliable transportation
CPR certification
Experience collaborating with healthcare professionals
Understanding of geriatric physical, emotional, social, and biological changes
Fluent in English (reading, writing, speaking)
Preferred Qualifications:
One year of experience as a Physical Therapy Assistant
