Role: A Physical Therapist (PT) providing skilled, patient-centered care in a home health setting.
Perform comprehensive patient evaluations and functional mobility assessments. Develop individualized therapy plans to improve strength, balance, and independence. Educate patients and caregivers on therapeutic exercises, injury prevention, and safe mobility strategies. Collaborate with an interdisciplinary team and ensure thorough EMR documentation. Work independently with travel to patient homes within the assigned territory. Qualifications & Skills: Active PT license in the state of practice. At least one year of clinical experience as a PT.
