Physical Therapist (Home-Based Rehab)
Provide compassionate, evidence-based rehabilitative care to patients in their homes. Conduct comprehensive assessments and create, implement, and adjust individualized treatment plans to improve mobility, safety, and quality of life.
Key Duties
- Assess patient needs and functional abilities
- Develop and update individualized treatment plans
- Deliver therapeutic interventions to support recovery and independence
- Educate patients and caregivers on care strategies and safe mobility
- Collaborate with nursing staff, social workers, and other clinicians
- Document services and patient progress in accordance with regulatory requirements
Qualifications & Skills
- Strong clinical assessment and rehabilitation planning skills
- Ability to provide patient-centered, compassionate care
- Knowledge of evidence-based physical therapy interventions
- Effective communication and teamwork with interdisciplinary providers
- Strong documentation skills and attention to compliance
- Comfort working independently in a home setting
