Physical Therapist (In-Home Rehabilitation)
Provide compassionate, high-quality in-home physical therapy for seniors and individuals with disabilities. Assess functional mobility, develop and implement individualized treatment plans, and deliver hands-on therapy to improve strength, balance, mobility, and pain management.
Key Duties
- Evaluate functional status and determine appropriate therapy interventions
- Design and implement personalized treatment plans
- Educate clients and caregivers on home exercise programs and safe mobility
- Collaborate with an interdisciplinary care team
- Document patient progress and update care plans as needed
- Support quality, compliance, and safety standards
Qualifications & Skills
- Active physical therapy licensure/eligibility to practice
- Strong clinical assessment and treatment planning skills
- Ability to provide effective hands-on therapy and patient education
- Excellent communication, teamwork, documentation, and safety focus
