Position Overview
We seek an Occupational Therapist (OT) to assess, teach, and deliver therapy interventions based on physician-directed home health plans. Responsibilities include performing initial and ongoing assessments of mobility, strength, and function; developing and updating treatment plans; documenting progress; and collaborating with healthcare team members, patients, and caregivers. The OT will supervise relevant personnel, complete OASIS assessments where permitted, and ensure documentation compliance to support optimal patient outcomes.
Qualifications & Skills
- Degree in Occupational Therapy from an accredited program
- Current, unrestricted state license as an OT
- Valid CPR certification
- Preferred: strong organization, communication, attention to detail, time management, and experience in home health therapy and OASIS documentation
Key Duties
- Conduct assessments and interpret results
- Create and modify individualized treatment plans
- Deliver occupational therapy services per physician orders
- Maintain accurate documentation and discharge planning
- Coordinate care with healthcare team and educate patients and caregivers
- Supervise staff as applicable and complete assessments within scope of practice
