Position Overview
Seeking an Occupational Therapist (OT) to assess, develop, and implement therapy interventions based on physician directives within a home health setting. The role involves conducting initial and ongoing evaluations of patients’ mobility, coordination, and functional abilities, and creating detailed care plans with measurable goals. The OT will deliver therapy services, document progress, modify treatment plans, and collaborate with healthcare team members, patients, and caregivers. Responsibilities include discharge planning, education, supervision of support staff, and completing assessments like OASIS where permitted. Candidates must hold a degree in Occupational Therapy, an active state license, CPR certification, and a valid driver’s license. Preferred skills include strong communication, organization, attention to detail, and experience with home health therapy and documentation.
