Position Overview
Seeking an Occupational Therapist (OT) to assess, teach, and deliver therapy interventions based on physician directives within the home health setting. Responsibilities include conducting initial and ongoing assessments of mobility, strength, and function, developing treatment plans, documenting progress, and modifying goals as needed. The OT collaborates with healthcare team members, educates patients and caregivers for discharge planning, and supervises other personnel such as OT Assistants and Home Health Aides where permitted. Additionally, the role involves completing OASIS assessments, maintaining accurate documentation, and ensuring compliance with policies.
Requirements:
- Degree in Occupational Therapy from an accredited program
- Current, unrestricted state license as an OT
- Valid CPR and driver’s license
