Join our healthcare team as a Home Health Nurse Practitioner providing compassionate, personalized care in patients’ homes.

Conduct patient assessments, develop care plans, and perform treatments like IV infusions and wound suturing.

Perform clinical procedures such as catheterization and phlebotomy, and provide patient education on disease management and lifestyle modifications.

Utilize EHR systems for documentation and coordinate with multidisciplinary teams for seamless care transitions.

Requirements: Current NP license, experience in critical care preferred, proficiency with EMR/EHR systems, and ability to perform complex procedures.

Details: Part-time, $32.06-$38.61/hr, with paid time off, hybrid remote work in Salem, OR.
