Seeking a Speech Language Pathologist to join our Home Care team, providing personalized evaluation and treatment in patients’ homes to enhance communication, cognition, and swallowing skills.
Responsibilities include interpreting medical prescriptions, conducting assessments, developing and adjusting evidence-based treatment plans, documenting progress, and educating patients and caregivers. Collaboration with interdisciplinary teams and participation in care planning are essential.
Required qualifications: Master’s degree in Speech-Language Pathology, CCC or CFY candidate, NY State license, and CPR certification. Preferred experience with dysphagia and neurogenic disorders.
