Seeking a Speech Language Pathologist for our Home Care team to provide personalized evaluations and treatments in patients’ homes, enhancing communication, cognition, and swallowing for safety and independence.
Responsibilities include interpreting medical prescriptions, conducting assessments, developing and adjusting evidence-based treatment plans, documenting progress, and educating patients and caregivers. Collaboration with interdisciplinary teams and participation in care planning are essential.
Qualifications include a Master’s degree, CCC from ASHA or CFY, NY State licensure, and CPR certification. Experience with dysphagia and neurogenic disorders is preferred.
