Seeking a Speech Language Pathologist for home care, providing personalized assessments and treatments to improve communication, cognition, and swallowing in patients’ homes.
Conduct evaluations using standardized tools and develop evidence-based, patient-centered care plans.
Adjust strategies based on progress and caregiver input, ensuring goals are practical for the home environment.
Document all interventions and progress, adhering to confidentiality and reimbursement standards.
Educate patients and caregivers on communication and swallowing techniques.
Collaborate with an interdisciplinary team to coordinate care and recommend assistive devices.
Maintain licensure and stay current with clinical practices, mentoring peers as needed.
Qualifications include a Master’s degree in Speech-Language Pathology, current NY state license, and CCC from ASHA or CFY status. Preference for experience with dysphagia and neurogenic disorders.
