Home Care Direct Support Role
Provide direct care to home care clients in their residences in accordance with each client’s plan of care.
Key Duties
- Assist with personal hygiene, transfers, and ambulation
- Support prescribed exercises and related care activities
- Help with meal preparation
- Offer companionship and provide light housekeeping
- Maintain accurate care documentation and records
- Report changes in client condition and notify appropriate parties of safety concerns
- Protect client privacy and maintain professional conduct
