Home-Based Physical Therapist

Role Overview: The Home-Based Physical Therapist provides comprehensive physical therapy services, including assessment, treatment planning, and interventions, adhering to professional and ethical standards. They contribute to program development, quality improvement, and interdisciplinary collaboration.

Key Responsibilities:

  • Conduct evaluations and treatments based on physician referrals for patients of all ages.
  • Administer therapies such as muscle strengthening, gait training, ADL training, and orthotic/prosthetic management.
  • Perform initial and ongoing patient assessments, documenting progress and adjusting treatment plans accordingly.
  • Develop goals for rehabilitation, evaluating patient progress towards short- and long-term objectives.
  • Complete accurate, timely clinical documentation per departmental and payer guidelines.
  • Educate patients and caregivers on activities and exercises, reinforcing verbal instructions.
  • Coordinate with healthcare providers, providing updates and modifying care plans as needed.
  • Manage appointment scheduling, travel logistics, and maintain portable therapy equipment.
  • Ensure compliance with departmental policies, safety, infection control, and quality standards.
  • Monitor insurance coverage limits and patient authorizations, communicating concerns to management.
  • Perform other duties as assigned, managing productivity independently.

Qualifications:

  • High School Diploma or GED required; Doctor of Physical Therapy (DPT) degree required after 2015.
  • Licensed Physical Therapist and valid driver’s license required.
  • Auto liability insurance and Basic Life Support (BLS) certification within 90 days.
  • Individuals must have reliable transportation for the role.
