Role: Periodic Home Site Supervisor
Responsibilities:
- Oversee day-to-day operations of person-centered home and community-based services.
- Ensure the delivery of safe, high-quality care aligned with individualized service plans.
- Supervise, coach, and support direct care staff.
- Coordinate staff schedules and maintain detailed documentation to ensure compliance with healthcare and safety regulations.
- Collaborate with families, case managers, and clinical partners to monitor progress, address concerns, and manage incidents.
