Role: Bilingual Licensed Professional Counselor providing psychotherapy and behavioral health services for seniors in long-term care, assisted living, and community settings.
Key Duties:
- Conduct comprehensive clinical and behavioral health assessments for older adults.
- Provide individual therapy and develop person-centered treatment plans.
- Collaborate with interdisciplinary facility teams to support resident care goals.
- Maintain accurate clinical documentation in an electronic health record (EHR).
- Deliver therapy services on-site and via telehealth.
Qualifications & Skills:
- Licensed Professional Counselor credentials.
