Responsibilities include:
- Performing comprehensive assessments: medical history, medication reconciliation, vital signs, physical exams
- Identifying diagnoses, documenting findings, and collaborating with primary care providers
- Recognizing urgent situations, educating members on health and medications, and addressing social determinants of health
Qualifications:
- Active licensure and certification (ANCC, AANP, NCCPA) in relevant fields
- Unrestricted license and driver’s license with reliable transportation
- Ability to navigate varied home environments and perform assessments
Preferred skills: clinical experience, proficiency with electronic medical records, strong communication skills with geriatric populations.
