Float Pool Registered Nurse (Home Health)
Provide high-quality, compassionate skilled nursing care to home health patients across a varied caseload by delivering assessments, medication and symptom management, and individualized care planning.
Key Duties
- Perform nursing assessments and monitor patient status in the home setting
- Manage medications, evaluate symptoms, and follow evidence-based protocols
- Develop and update care plans in collaboration with an interdisciplinary team
- Educate patients and families on care, safety, and treatment plans
- Provide continuity and safety while flexing between teams/territories as needed
- Document care accurately in the EMR and maintain clinical compliance
Qualifications & Skills
- Current RN license
- Strong clinical judgment, assessment, and patient-centered communication
- Ability to work independently in the home environment and adapt to varied caseloads
- Proficiency with EMR documentation and adherence to established protocols
