Role Overview:
Seeking a registered nurse to provide in-home assessments, patient education, and care coordination in a community-based setting. The nurse will identify treatment gaps, facilitate referrals, and support members with condition management, medication reviews, and resource connections, ensuring a seamless health journey across care settings.
Key Responsibilities:
- Conduct home visits to evaluate health status
- Identify barriers in treatment plans
- Educate patients for self-management
- Make appropriate referrals
- Maintain communication with members via in-person or phone interactions
- Support with medication reviews and resource connections
Qualifications & Skills:
- Unrestricted RN license in Washington
- Basic Life Support certification or ability to obtain
- At least 1 year of clinical experience, preferably in home care
- Excellent communication, computer, and software navigation skills
- Reliable transportation and residency near Seattle
Preferred Qualifications:
- Bachelor of Science in Nursing (BSN)
- Certified Case Manager (CCM)
- Experience in discharge planning, utilization review, or risk management
- Bilingual in English and Spanish
