Family Medicine Physician – Live Well Homecare

Seeking a Family Medicine Physician to provide comprehensive, holistic primary care for patients of all ages, including diagnosis, treatment, preventive care, and minor procedures.

Perform assessments, manage chronic conditions, and coordinate specialist care.

Provide pediatric and prenatal/postnatal care where applicable.

Demonstrate strong communication, cultural sensitivity, and organizational skills.

Require Illinois Medical license or eligibility, Board Certification or Eligibility, and MD or DO degree.

Work Monday–Friday, 8:00am–5:00pm, with clinical and scholarly responsibilities. Benefits include health insurance, 401(k) match, paid time off, tuition reimbursement, and more. Salary ranges from $225,000 to $295,000 annually.
