Family Medicine Physician – Live Well Homecare

Seeking a Family Medicine Physician to provide comprehensive outpatient primary care for patients of all ages in a growing, established practice.

Deliver primary care including assessment, diagnosis, treatment of acute and chronic illnesses, and preventive services.

Manage chronic conditions such as diabetes, hypertension, and asthma; perform physical exams, order tests, prescribe medications, and provide patient education.

Collaborate with healthcare team, conduct in-office procedures, and participate in utilization review and infection control.

Take after-hours calls on a rotating basis.

Qualifications include Board Certification or eligibility, valid medical license, DEA license, strong clinical skills, and excellent communication.
