Role Overview: A Nurse Practitioner (NP) or Physician Assistant (PA) is sought to provide comprehensive family medicine services in a team-based primary care setting.
Key Responsibilities: Conduct patient evaluations, manage acute and chronic conditions, order and interpret diagnostic tests, prescribe medications, coordinate referrals, and deliver preventive care and patient education. Collaborate with healthcare team members to ensure continuity of care and maintain accurate clinical documentation.

Qualifications & Skills: Active NYS license as an NP or PA, with relevant certifications. Compensation: Competitive salary starting at $125,000 plus benefits.
