Family Care Physician – Live Well Homecare

Seeking a permanent, board-certified Physician (Internal Medicine, Family Medicine, or Med/Peds) to provide primary care in an established Philadelphia FQHC serving diverse community members.

Provide direct outpatient care, ensure continuity for inpatient services, and participate in after-hours rotation.

Focus on wellness promotion, chronic disease management, and meeting community-specific health needs.

Precept learners and engage in educational activities.

Requirements include PA licensure, board certification or eligibility, excellent communication skills, and experience working in inner-city settings.
