The Advanced Practice Provider (APP) provides comprehensive outpatient primary care under supervision at the Prineville Clinic, serving patients of all ages.
Responsibilities include evaluation, diagnosis, treatment, documentation, and coordination of care within a Medical Home model. Creates integrated treatment plans incorporating behavioral health, pharmacy, therapy, nutrition, and surgery; monitors patient progress and refers to specialists as needed. Works a minimum of 36 hours of patient contact weekly, participates in shared call rotation including evenings, weekends, and holidays. Collaborates with multidisciplinary teams to deliver evidence-based, personalized care, ensuring quality outcomes and patient satisfaction. Maintains documentation, billing, and compliance with policies and licensing requirements. Engages in ongoing education, quality improvement, and adheres to standards of professionalism and confidentiality.
Qualifications include a master’s degree in Physician Assistant Studies or Nursing, valid licensure, certifications, and experience in outpatient family medicine. Preferred candidates have 2+ years of experience, familiarity with Medical Home models, and Epic EHR experience.
