Advanced Practice Provider position providing outpatient primary care at a Family Care Clinic in Prineville, OR.
Under supervision, evaluates, diagnoses, treats, and documents patient encounters across all ages. Coordinates medical, behavioral health, nursing, pharmacy, nutrition, and surgical services within a Medical Home model to optimize outcomes. Develops comprehensive treatment plans, monitors patient progress, and refers to specialists as needed. Works a minimum of 40 hours/week, including participation in shared call rotations, evenings, weekends, and holidays. Collaborates with a multidisciplinary team supported by behavioral health, pharmacy, and care management teams. Practices evidence-based family medicine, prescribes medications, and ensures timely documentation and reporting. Maintains licensure, certifications, and compliance with organizational policies and regulations. Requires a Master’s degree in Nursing or Physician Assistant Studies, valid licensure, certification, and outpatient family medicine experience.
