Role Overview: A Physical Therapist acting as a Digital Health Specialist, primarily working in outpatient settings with a hybrid model of in-clinic and remote care.
Key Responsibilities:
- Develop and deliver personalized treatment plans for patients with orthopedic and neurologic conditions.
- Provide virtual care through telehealth platforms, including evaluations, assessments, and remote monitoring.
- Objectively measure patient outcomes using specialized software and document all clinical activities accurately.
- Support operations by facilitating digital health services and ensuring patient care and staffing needs are met.
- Coordinate with clinicians and serve as a liaison to optimize remote service delivery and program growth.
- Maintain regular reporting and analyze regional service metrics; participate in program development and marketing initiatives.
Qualifications & Skills:
- Licensed in physical therapy, with a diploma from a CAPTE-approved program.
- Experience in outpatient settings; telehealth experience preferred.
- Strong dedication to providing high-quality patient care.
