CORE Licensed Provider

The licensed CORE provider offers face-to-face mobile services in community settings for individuals with mental health and substance use disorders.

Key responsibilities include

delivering person-centered, goal-oriented support and solution-focused interventions

identifying treatment strategies to reduce symptom impact

providing supportive counseling to individuals and families

facilitating strengths-based planning and treatment

helping clients recognize and manage triggers

implementing evidence-based psychotherapeutic interventions

documenting care accurately and attending supervision and training

Required qualifications include

licensed mental health professional with at least 3 years’ community experience

strong interpersonal, communication, and collaboration skills

familiarity with recovery principles, health programs, and electronic health records

current license, valid NY driver’s license, and bilingual or lived experience preferred
